Job Summary
Buckeye Commons Care is seeking a highly motivated and results-driven
Healthcare Community Liaison to join our team in
Kent, Ohio
. As a
Healthcare Community Liaison
, you will be responsible for building relationships with referral partners, educating them on our services, and helping patients and families connect with the right level of post-acute care.

Responsibilities
Build and maintain strong relationships with hospitals, physician offices, and other referral partners to generate referrals and grow market share
Develop and execute a territory development plan to increase census and revenue
Identify opportunities to expand our services and partnerships throughout the community
Cultivate relationships with referral sources, including case managers, discharge planners, and other healthcare professionals
Develop and maintain a comprehensive understanding of Sweetwater Care's services and post-acute care options
Provide education and training to referral partners on Sweetwater Care's services and benefits
Identify and capitalize on opportunities to improve patient outcomes and reduce healthcare costs
Requirements
Competitive salary
