Job Description

HarmonyCares, a physician-led home-based primary care and palliative care organization, is seeking a Community Liaison for Marketing & Business Development in Renton, WA. In this role, you will build and nurture relationships with community partners, referral sources, and senior care organizations to expand access to HarmonyCares' services. You will represent our mission at events, educate providers and families about home-based care options, and collaborate closely with clinical teams to ensure smooth referrals and improved outcomes for medically complex, home-limited patients.

Responsibilities

  • Build and maintain relationships with community partners, referral sources, and senior care organizations to grow patient volume
  • Represent Harmony
  • Cares at community events, health fairs, and professional meetings to promote home-based primary care services
  • Develop and execute local outreach and marketing plans to increase brand awareness in the Renton area
  • Collaborate with clinical and care management teams to coordinate smooth referrals and patient onboarding
  • Track outreach activities, leads, and conversion metrics using CRM or similar tools
  • Educate community providers, caregivers, and families about Harmony
  • Cares programs and eligibility criteria
  • Provide timely market feedback to leadership on trends, competitor activities, and partnership opportunities
  • Support creation and distribution of marketing materials tailored to community partners and patients
  • Participate in interdisciplinary team meetings to align outreach with patient care goals
  • Ensure all outreach and marketing practices comply with healthcare regulations and organizational policies
