The Community Integration Coordinator plays a crucial role in facilitating the transition of individuals with varying needs back into the community. This position is responsible for providing services that support recovery, stability, and independence among participants, ensuring they can navigate community resources effectively and live fulfilling lives. Collaborating with various stakeholders, the coordinator will develop comprehensive integration plans that address individual needs and preferences.
Position Title: Community Integration Coordinator
Position Type: Full Time; 35 Hours/Weekly

Department: Community Services
Responsibilities:
- Provide clinical and administrative supervision to program staff
- Provide ongoing comprehensive assessments including biopsychosocials and substance use
- Actively participate in performance improvement activities for optimal client care
- Participate in Utilization Review to improve strategies and operations for the benefit of consumer care
- Direct and coordinate housing placement by maintaining working relationships with housing providers
- Maintain relationships with adult educuation programs and coordinate services for consumers
- Provide crisis intervention as needed
- Supervise discharge planning and referral services.
- Participate in personnel actions including recruitment
- Provide direct clinical care and case management services to clients and their families
- Master's Degree in Rehabilitation, Health and Human Services or related field
- Two years of experience working with individuals with severe persistent mental illhess
- Experience in community outreach, social services, or related area
- Strong interpersonal and communication skills
- Knowledge of mental health and community resources preferred
- Valid NJ Driver’s License with less than 6 points
