Job DescriptionJob Title: Senior Case Manager (Scattered Site)\n\nEducational Requirements: Bachelor's Degree Required with relevant experience.\n\nQualifications and Experience: Experience working with mental health, crisis intervention, and managing trauma-triggered behavior. Excellent time management skills; stress-resistant, flexible; team player; open to feedback; creative; exceptional organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone.

Preferred knowledge of AWARDS.\nHours: Full-Time, 35 hours per week, Monday-Friday Schedule.\n\nPay/Salary: $50,000 annually \n\nScope of Position: \nUnder the supervision of the Program Coordinator, the Case Manager provides supportive services to formerly homeless adults living in scattered-site apartments throughout the Bronx. The population served includes individuals diagnosed with serious mental illness, seniors, veterans, medically frail individuals, and those with a forensic history.

The Case Manager meets individually with clients in their apartments, completes required documentation promptly and in accordance with agency policy, and regularly conducts fieldwork.\n\nCase Manager Job Responsibilities:\n\n\n Deliver comprehensive case management services, including housing assistance, service referrals, benefits maintenance, client escorting, assessments, and coordination with external providers\n\n\n\n These services are primarily provided in the client's apartment and in community settings.\n\n\n\n Coordinate with the clinical High-Risk Coordinator to provide case management and interventions.\n\n\n\n Assist in identifying strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system.\n\n\n\n Use Motivational Interviewing techniques to help clients develop skills needed to maintain housing and develop natural supports.\n\n\n\n Timely completion of case notes (within 48 hours) and other required documentation for every client contact as per agency policy.\n\n\n\n Provide crisis intervention, as necessary, utilizing de-escalation techniques.\n\n\n\n Compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation, and incident reporting.\n\n\n\n Ability to work with consumers/residents, families, community tenants, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.\n\n\n\n Conduct apartment inspections on multiple floors, utilizing stairs.

Escort clients throughout the community using public transportation.\n\n\n\n Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors.

Completes incident reports in accordance with Bridge policy.\n\n\n\n Accounts for consumers, performs diligent searches, and files missing person reports on consumers not accounted for in accordance with policy and procedure.\n\n\n\n Attend staff meetings, supervision, and trainings as required.\n\n\n\n Any other duties as may be assigned, including the potential of supporting other programs within the agency portfolio.\n\n\n\nFull-time employees receive a generous benefits package, including:\n\n\n Competitive health, vision, and dental plans.
