Your Role as the MDS Coordinator (LPN) Skilled NursingAmericare Senior Living is seeking a detail-oriented and clinically knowledgeable MDS Coordinator (LPN) to support accurate assessments, regulatory compliance, and quality resident outcomes within our skilled nursing community. In this role, you’ll coordinate the MDS process in accordance with federal and state guidelines while also supporting direct resident care, interdisciplinary collaboration, and a safe, home-like environment.

Your work plays a critical role in care planning, reimbursement accuracy, survey readiness, and overall quality of life for residents. Summary The MDS Coordinator (LPN) is responsible for coordinating and completing the MDS process as required by federal and state regulations while ensuring accurate, timely, and compliant clinical documentation. This role collaborates closely with the Director of Nursing, Administrator, and interdisciplinary team (IDT) to maintain facility standards, support inspections, and promote high-quality resident care.

This role may also be known as: MDS Coordinator (LPN), MDS Nurse (LPN), Clinical Reimbursement Nurse (LPN), Assessment Coordinator, or Case Mix Nurse (LPN).What You’ll Do Coordinate and complete MDS assessments in accordance with federal and state guidelinesEnsure admission assessments, nursing notes, over-all care plans, and required documentation are accurate and current per facility, ASI, and regulatory standardsComplete assigned MDS sections and required assessments, including but not limited to:Skin, contracture, fall, incontinence, catheter, and side rail assessmentsRAP summaries for urinary incontinence, falls, indwelling catheters, feeding tubes, dehydration, oral and dental care, pressure ulcers, psychotropic medications, and physical restraintsReview, update, and maintain resident care plans based on assessment findingsParticipate in Medicare meetings, care plan meetings, and interdisciplinary team discussionsAssist the Director of Nursing with maintaining compliance with state, federal, and Americare requirementsSupport survey readiness and participate in inspections as neededEnsure medication systems meet resident needs and comply with state regulations and pharmaceutical guidelinesCommunicate significant changes in resident condition to the Administrator and nursing leadership, including hospital transfers or other critical eventsAssist with resident admissions, placements, and room transfers in coordination with the IDT teamSchedule, participate in, and maintain records for nursing staff meetings as neededReview incident reports routinely and ensure appropriate documentation and follow-upAssess nursing supply and equipment needs and ensure adequate inventory levelsProvide on-call support to staff as needed for nursing-related mattersPerform other duties as assigned by the AdministratorWhat You BringEducation Graduate of an accredited school of nursingLicensure / Certification Current Licensed Practical Nurse (LPN) license requiredMaintains current state registration and required continuing educationExperience & Knowledge Three (3) to five (5) years of healthcare experience preferredOne (1) or more years of clinical experience in a Skilled Nursing Facility preferredTwo (2) or more years of long-term care or assisted living experience preferredKnowledge of Medicare reimbursement, RUGs, and state and federal regulationsSkills Strong attention to detail, organization, and analytical skillsAbility to collaborate effectively with interdisciplinary teamsClear and professional communication skillsCommitment to accuracy, compliance, and resident-centered carePhysical Demands Lift up to 25 lbs. and assist with resident transfers as neededPush, pull, and maneuver up to 100 lbs., including resident mobility assistanceManeuver resident wheelchairs weighing up to 350 lbs.
