The Role

• Represent the Outpatient Care Management in their absence
• Ensure the creation of care processes, considering individual care levels
• Conduct nursing visits and initial consultations
• Perform direct care duties within the outpatient service
• Maintain meticulous documentation of care provided
• Collaborate with the Care Manager and regional Quality Management

What You'll Need

• Completed professional training in geriatric or general nursing
• Minimum of two years' professional experience, with at least nine months in outpatient care
• Demonstrated empathy and leadership skills
• Proficiency in IT systems and strong communication abilities
• Flexibility, an open mindset, and a valid driver's license (Class B)
• Enjoyment and dedication to working with elderly and care-dependent individuals
