We are looking for two Trusted Assessors to help ensure people leave hospital safely, quickly and with the right support in place.

Based within the Integrated Discharge Hub at Hairmyres University Hospital, you will work closely with care homes, health professionals, social work staff, individuals, and their families to support smooth and timely hospital discharges. The role aims to reduce unnecessary delays, improve outcomes for people using services, and make the discharge process more straightforward and effective.

The Trusted Assessor plays an important coordinating role, providing advice, support, and oversight throughout the assessment and discharge process. Acting as a key link between partners, you will help ensure decisions are made promptly and that people receive the right care in the right place at the right time.

This is a temporary one-year post, subject to review and evaluation.

Key Responsibilities

  • Undertake high-quality assessments and reassessments on behalf of residential and nursing care homes to support safe and timely discharge from hospital.

  • Work closely with hospital teams and partner agencies to coordinate communication and provide a single point of contact for those involved in a person's discharge.

  • Support care homes to complete their own assessments, ensuring agreed timescales are met and avoidable delays are minimised.

  • Act as the main point of contact for Care Home Managers, providing advice, coordinating assessments and discharge arrangements, offering professional support and reassurance, and ensuring timely follow-up throughout the discharge process.

  • Work collaboratively with care homes to promote consistent practice and the use of an agreed assessment tool.

  • Where required, complete assessments using care home-specific documentation while ensuring consistency with agreed quality standards and discharge requirements.

  • Communicate assessment outcomes clearly and effectively with individuals, families and multidisciplinary partners.

  • Maintain accurate and secure electronic records in line with data protection and information governance requirements.

  • Identify opportunities to improve processes and contribute to service development and continuous improvement.

  • Record activity and performance information accurately to support monitoring, evaluation and reporting.

  • Promote awareness and understanding of the Trusted Assessor role across hospital and care home settings.

  • Contribute to local and national service development activities, including participation in the National Community of Practice for Trusted Assessors
