Job Description

RN Hospice Clinical Liaison

Position Responsibilities:

·        Develop, expand and maintain strong referral-source relationships across the assigned market.

·        Represent hospice services professionally with referral partners, patients, families and community partners.

·        Use clinical knowledge to educate referral sources, patients and families on hospice care, eligibility, services and transition planning.

·        Identify new referral opportunities and strengthen existing accounts through consistent outside field activity.

·        Work closely with internal clinical and operations teams to support appropriate referrals and smooth transitions into care.

·        Current, unencumbered Registered Nurse license required.

·        Hospice and/or Home Health clinical liaison, hospice admissions, healthcare sales, marketing, business development or referral-development experience strongly preferred.

·        Minimum one year of healthcare sales, marketing or liaison experience required; two or more years preferred.

·        Hospice and/or palliative care marketing or liaison experience strongly preferred.

·        Established Boston / South Shore hospital, facility and referral-source contacts strongly preferred.

·        Local market knowledge, job stability and proven, verifiable performance are must-haves.

·        Ability to establish and expand relationships with diverse healthcare referral sources.
