Job Description

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Clinical Account Executive to grow our in-home care services for seniors and adults with chronic conditions. In this role, you will build and manage relationships with physicians, hospitals, and community partners, serving as a clinical liaison to coordinate smooth patient referrals and admissions. You'll educate providers on our skilled nursing and therapy services, collaborate with our interdisciplinary teams, and use data and CRM tools to track performance. Responsibilities

  • Develop and manage relationships with physicians, hospitals, and community partners to drive home health referrals
  • Serve as clinical liaison, assessing referral needs and coordinating smooth patient admissions to home health services
  • Educate providers and community partners on Center
  • Well Home Health services, capabilities, and care pathways
  • Collaborate with clinical and operations teams to ensure high-quality, patient-centered in-home care
  • Track performance metrics, referral trends, and conversion rates; report results and identify growth opportunities
  • Represent Center
  • Well at community events, meetings, and outreach activities to build brand awareness
  • Maintain accurate account activity and pipeline information in CRM and other systems
  • Support continuous improvement in patient experience, clinical quality, and referral partner satisfaction
