We're hiring on behalf of a highly reputable medical center that seamlessly integrates outpatient and inpatient care through close collaboration with an affiliated hospital. This dynamic environment focuses on holistic patient care, combining diagnostics, therapy, and long-term support for approximately 10,000 patients annually across multiple disciplines.

• *The Role**
• Oversee structured treatment programs for patients with Asthma, COPD, Coronary Artery Disease, and Diabetes Mellitus.
• Provide comprehensive and empathetic medical care to patients.
• Ensure meticulous documentation of all program-relevant data and monitor therapy intervals.
• Collaborate effectively in an interdisciplinary team to actively shape patient treatment plans.
• Deliver high-quality diagnostics and therapy in accordance with current medical standards and guidelines.

• *What You'll Need**
• Completion of medical studies with a recognized specialization in General Medicine or Internal Medicine.
• Desirable knowledge in Disease Management Programs (DMP) and ultrasonography.
• Willingness to conduct house and home visits is a plus.
• A genuine passion for patient interaction and a respectful approach.
