Department: Acute Rehab

Schedule: Rotating Schedule | PRN

• Lead the care team to ensure a seamless, goal-oriented recovery for every patient.
• Execute personalized care plans and monitor clinical progress to drive successful outcomes.
• Advocate for patient safety by processing orders and communicating vital status changes.
• Coordinate comprehensive discharge paths to ensure patients transition safely back to their lives.

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
