Schedule: Full-Time | Monday-Friday | 8:00am-4:30pm
Guide evidence-based heart failure therapy by initiating and adjusting targeted medications to achieve optimal therapeutic dosing and improve patient survival.
Safeguard care transitions by reconciling inpatient and outpatient medication plans, eliminating harmful drug interactions and keeping cardiac regimens safe and effective.
Track essential kidney and electrolyte laboratory trends proactively to prevent treatment-related complications and sustain safe therapy progression.
Empower patients and caregivers through personalized education on symptom monitoring, self-care routines and daily regimen management to reduce avoidable hospital visits.
Partner with cardiologists, specialized nurses and primary care clinicians under collaborative practice models to deliver unified, high-impact cardiovascular care.
Licensure / Certification / Registration:
- Licensed Pharmacist credentialed from the Oklahoma State Board of Pharmacy obtained within 3 Months (90 days) of hire date or job transfer date required.
Education:
- Bachelor's or advanced degree in Pharmacy required.
