Occupational Therapist - Monday to Friday / No Weekends

As an Occupational Therapist in Home Care, you will evaluate and treat individuals with various physical and cognitive impairments. You will develop treatment plans tailored to each patient’s unique needs, using a holistic approach to promote recovery and enhance daily living skills.

Responsibilities

• Evaluate patients’ functional abilities and occupational therapy needs in the home setting.
• Develop and implement individualized treatment plans focused on improving independence and quality of life.
• Provide skilled OT interventions addressing ADLs, mobility, strength, coordination, endurance, and safety.
• Assess the home environment and recommend appropriate adaptive equipment and safety modifications.
• Educate patients, families, and caregivers on therapeutic exercises, techniques, equipment, and safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely clinical documentation.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Communicate changes in patient condition and escalate clinical or safety concerns appropriately.
• Participate in care planning and discharge planning as needed.
• Maintain compliance with applicable regulatory, professional, and agency requirements.
• Travel to patients’ homes throughout Philadelphia and surrounding counties.
• Perform other duties as assigned.

• Graduate of an accredited occupational therapy program
• Current active state licensure as an Occupational Therapist in Pennsylvania
• A minimum of one year of experience in a clinical or post-acute healthcare setting
• Prior experience in home health care is highly desirable
• Strong assessment, clinical, and organizational skills
• Ability to create personalized treatment plans and adapt techniques as necessary
• Valid Pennsylvania Driver's License and reliable transportation
• Excellent interpersonal and communication skills
• Ability to work collaboratively with multidisciplinary teams
• Current professional liability insurance required
• Ability to follow oral and written instructions effectively
• Proof of Influenza Vaccination or an approved Medical or Religious exemption.
