Full-time, mission-driven role supporting older adults through comprehensive therapy, rehabilitation, and interdisciplinary care. Our ideal candidate will need to work flexibly between clinic locations and provide care directly in participants’ homes.
Job Overview
We’re seeking a dedicated Physical Therapist to provide high-quality physical therapy services to participants enrolled in a local PACE program. The ideal candidate is skilled in assessment, care planning, therapeutic interventions, and collaborative teamwork. This role focuses on improving participant safety, mobility, independence, and overall quality of life through evidence-based therapy and coordinated care.
Responsibilities
• Conduct initial assessments and quarterly reassessments to evaluate functional status and establish treatment goals
• Develop and implement individualized Plans of Care in collaboration with the Interdisciplinary Team
• Deliver therapeutic interventions and monitor progress for assigned participants
• Apply treatment modalities effectively, understanding indications and contraindications
• Assess participant needs for equipment, orthotics, and prosthetics and assist with procurement
• Participate in physical therapy maintenance programs and coordinate with therapy staff
• Educate participants, caregivers, and families on exercises and safety techniques
• Collaborate with external physical therapists when participants are hospitalized or institutionalized
• Complete timely and accurate documentation in the electronic medical record
• Contribute to program development and quality improvement initiatives
• Maintain clinical competency and adhere to professional, departmental, and regulatory standards
• Support safety practices, emergency preparedness, and proper body mechanics
• Meet productivity, quality, and customer service goals
• Participate in staff in-services and training as needed
• Promote participant rights, dignity, confidentiality, and independence
• Build strong working relationships with staff, contractors, and the community
• Other duties as assigned
Requirements to apply
• Bachelor’s degree in Physical Therapy from a CMS-approved institution
• Active California Physical Therapy license
• Minimum one year of experience working with a frail or elderly population
• Valid California driver’s license, personal vehicle, and auto liability insurance
• CPR and First Aid certification (required within 6 months of hire)
