Social Worker Case Management

• Location: Indianapolis, IN
• Facility: Ascension St. Vincent
• Department/Specialty: Adult Inpatient, Ortho
• Schedule: Monday-Friday, Day shift, 8:00AM-4:30PM

• Assess complex emotional and social barriers to develop personalized care plans that address each patient’s unique needs and preferences.
• Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care.
• Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person-centered.
• Empower patients and families through education on financial and community resources, helping them build the stability needed to manage their diagnosis.

Licensure / Certification / Registration

• Required Credential(s):
  • None Required.
• Preferred Credential(s):
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC). ACM also accepted.
  • Certified Social Worker.

Education

• Bachelor's degree in Social Work required.

Work Experience

• 3 years of experience preferred.

Additional Preferences

• Must have a Social Worker license.
• Masters degree in Social Work required.
