RN Nurse Consultant Social Adult Day Care

Grace Community Care & Homes is developing its Adult Day Programs department and seeks a RN Nurse Consultant Social Adult Day Care. The role will contribute to person-centered services, reliable operations and accurate documentation.

Core responsibilities

Consult on health profiles, observations, staff training, escalation procedures and care planning for the social day program.

Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement.

Program readiness

This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope.

Location and schedule

New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. This is a part-time role. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program's authorized scope.

Active NJ RN license and relevant community or adult-care experience; capacity to provide the consultation required by the final program. Applicants should provide relevant credentials, experience, availability and a resume. Final regulatory eligibility will be verified before appointment or service delivery. Pay: $45-$55 per hour, based on qualifications and relevant experience.

Employment type: Part-time. Weekly hours and start date will be discussed during hiring; this planned program role is contingent on required approvals and staffing readiness.
