Job Description

A Thriving Mobile Wound Care Practice

Location: Boerne, TX (serving patients throughout Texas)

Employment Type: Full-Time

Reports To: Clinical Director / Practice Manager

About the Role

We are seeking a Wound Care Nurse to provide mobile, in-home advanced wound care to patients

across Texas as part of our Woundlocal clinical team. This role combines hands-on clinical skill with

compassionate patient care, bringing high-quality wound management directly to patients who need it

most.

Key Responsibilities

Clinical Care

• Perform wound assessments, debridement, and advanced dressing changes per physician orders

and clinical protocols

• Travel to patients' homes or care facilities to deliver mobile wound care visits

• Monitor wound healing progress and escalate concerns to providers as needed

• Educate patients and caregivers on wound care, infection prevention, and home management

Documentation & Coordination

• Maintain accurate, timely clinical documentation in the EMR

• Communicate findings and care plans clearly to supervising providers

• Coordinate with referral sources and care teams to ensure continuity of care

• Order and manage wound care supplies and DME as needed for patient care

Compliance & Quality

• Maintain strict confidentiality of patient health information in accordance with HIPAA

• Follow clinical protocols, infection control standards, and practice policies

• Participate in quality assurance reviews and audits as requested

Required

• Active, unencumbered LVN or RN license in the State of Texas

• Minimum 2 years of clinical nursing experience; wound care experience strongly preferred

• Comfortable traveling to patients' homes and care facilities throughout the service area

• Working knowledge of HIPAA and clinical documentation standards

• Daily experience working in an EMR

• Strong clinical judgment and high attention to detail

• Reliable transportation and a valid driver's license

• Professional appearance and demeanor

• Comfortable using Apple computers/tablets in the field

Preferred

• Certification in wound care (e.g., WCC, CWCA, or similar)

• Experience with mobile, home health, or hospice nursing

• Bilingual candidates encouraged to apply

• Compensation: $28–$40 per hour, depending on experience and licensure

• Schedule: Monday–Friday, full-time (40+ hours/week); field-based with some flexibility
