Job Description
Grace Community Care & Homes is developing its Home and Community Care department and seeks a Home Care Scheduling Coordinator. The role will contribute to person-centered services, reliable operations and accurate documentation.
Core responsibilities
Match clients and qualified staff, monitor coverage, manage call-outs, reconcile schedules with authorizations and coordinate visit documentation.
Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement.
Program readiness
This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope.
Location and schedule
New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. Part-time; assignment and weekly schedule discussed during hiring. Clinical services begin only at approved locations and within authorized scope.
Home-care scheduling experience, dependable communication and familiarity with authorization and electronic visit workflows. Applicants should provide relevant credentials, experience, availability and a resume. Final regulatory eligibility will be verified before appointment or service delivery. Pay: $20-$25 per hour, based on qualifications and relevant experience.
