The case manager supports referrals, care coordination, resource linkage, and follow-up with internal programs and community partners. All work must follow the assigned program manual, role expectations, approved service model, and the Psychosocial Rehabilitation Model when it applies. The role focuses on social determinants of health, including housing, benefits, transportation, food, medical care, education, employment, safety, and other community needs.
The case manager completes assigned daily tasks, monitors their caseload, supports program KPIs, uses agency dashboards and data to guide their work, and identifies barriers that affect client access, engagement, outcomes, and program performance. The case manager also provides identified clinical and/or medical linkage to interventions as outlined by the program manual, assigned role, and scope of work. The case manager may have other duties assigned by the Program Manager.

- Meets annual performance expectations for productivity, quality, documentation, client engagement, role-specific KPIs, and 1,200 clinical interaction hours each year unless adjusted based on role, leave, program need, or organizational need. Staff are expected to understand the KPIs listed for their role and program, track their progress towards kpis, follow program manual expectations, and ask for support when barriers affect performance.
- Caseload Management: Manages the assigned caseload by monitoring client engagement, follow-up, service needs, outcomes, documentation, care coordination tasks, and services within the assigned role related to the program manual. Staff are expected to know who is on their caseload, what each client needs are, and when barriers need to be escalated.
- Documents client contacts and work completed on behalf of clients according to the assigned program manual, documentation standards, and Electronic Health Record (EHR) requirements. Uses EHR data and agency provided dashboards to identify trends, prioritize outreach, re-engage clients, and share progress or concerns with the Program Manager.
- Evaluates client needs, coordinates referrals to internal programs and external resources, and helps clients access medical, behavioral health, social, educational, housing, financial, transportation, food, employment, legal, and community supports as outlined by the program manual and assigned role.
- Coordinates care with the client’s treatment team, primary care provider, internal programs, external resources, support systems, and family members as appropriate and as allowed by the program manual, client consent, and agency policy.
- Participates in annual performance reviews and works with their manager to track KPI progress, identify barriers, and create action plans that align with the assigned program manual, role expectations, and program goals.
- Provides individual and group interventions using approved evidence-based models and best practices listed in the assigned program manual, an example is Motivational Interviewing, when they apply to the program, role, and client need. Interventions should support client engagement, housing stability, medical appointments, healthy living goals, and other goals identified in the service plan.
- Attends treatment team meetings, case review meetings, and other required clinical review meetings as outlined by the program manual and manager. Staff share updates, make recommendations for the treatment plan, discuss barriers, and provide crisis support and emergency services when required.
- Completes needs assessments, service plans, referrals, linkage, monitoring, follow-up, and appropriate escalation of barriers according to the assigned program manual, role expectations, and client needs.
- Uses program resources responsibly and understands how daily case management work affects the program budget, service use, staffing capacity, transportation use, and program sustainability. Staff follow program manual guidance when using program resources.

- Actively participates in audits of clinical sessions and supervision with their manager and/or director. This may include in-person observation, audio review, video review, chart review, and discussion during supervision. These reviews are used to support learning, improve service quality, confirm use of the program manual and approved service model, and help staff meet role expectations.
