Social Worker - Mental Health

Our client in South East London is looking for an experienced Social Worker to join a well-established Community Mental Health Team.

This is a fantastic opportunity to work within an integrated service alongside NHS professionals, supporting working-age adults with a diagnosis of psychosis. The team is known for its strong culture, excellent morale, and collaborative approach, offering a supportive environment for practitioners looking to further develop their mental health expertise.

Key Responsibilities

• Hold a caseload and act as Care Coordinator, providing ongoing support and mental health monitoring.
• Complete Care Act assessments, including needs and carer assessments.
• Review care packages, placements, and funded support services.
• Undertake risk assessments and Mental Capacity Assessments.
• Participate in the team duty rota and respond to urgent enquiries.
• Lead discharge planning and present cases to relevant funding panels, including Care Act and Section 117 requirements.
• Act as an Enquiry Officer for Adult Safeguarding investigations.
• Work collaboratively with multidisciplinary colleagues and partner agencies including housing, police, and substance misuse services.

Requirements
To be considered for this role, you will need:

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
