Job DescriptionClinical Lead – Nurse Practitioner (NP) / Physician Assistant (PA) | PACE Program\nJoin our team and make a meaningful difference in the lives of older adults.\nWe are seeking a compassionate and experienced Clinical Lead Nurse Practitioner to join our PACE (Program of All-Inclusive Care for the Elderly) team.

This is an exciting opportunity to provide comprehensive, coordinated care to older adults while working alongside an interdisciplinary team dedicated to helping participants remain healthy, independent, and in the community.\nAs a Clinical Lead NP, you will partner with primary care physicians and interdisciplinary team members to oversee and coordinate participant care, manage acute and chronic conditions, and develop individualized care plans.\nWhat you’ll do:\n\n Provide comprehensive primary and acute care to PACE participants.\n Conduct annual wellness and routine chronic care assessments.\n Assess, diagnose, and manage acute and chronic medical conditions.\n Perform post-hospitalization visits and coordinate transitions of care.\n Develop and oversee individualized participant care plans.\n Educate participants and families on disease management, prevention, and healthy aging.\n Coordinate care with specialists, community providers, and the interdisciplinary team.\n Maintain accurate and timely documentation in the EHR.\n Provide clinical leadership and support at the assigned PACE site.\n Collaborate with physicians, nurses, social workers, therapists, and other healthcare professionals to deliver person-centered care.\n\nSchedule: Monday–Friday | 8:00 AM–5:00 PM | 40 hours/week\nWhat we’re looking for:\n\n Licensed and certified Nurse Practitioner.\n Graduate of an accredited NP program.\n Strong clinical judgment and communication skills.\n Experience in primary care, geriatrics, complex care, or chronic disease management preferred.\n Passion for providing compassionate, coordinated care to older adults.\n\nWhy PACE?\nPACE brings medical, behavioral health, social, and supportive services together around the needs of each participant.
