Job Summary

Summit Healthcare Staffing is hiring on behalf of a client who is seeking a compassionate and detail-oriented Bilingual Registered Nurse (RN) to join their Intake team at a growing home health organization in Brooklyn, NY. This is an excellent opportunity for an experienced nurse who enjoys coordinating patient care, collaborating with interdisciplinary teams, and ensuring patients receive a seamless start to home health services.

This position begins on-site in Brooklyn, with the opportunity to transition into a remote role based on performance and training.

Responsibilities

  • Review and process incoming home health referrals.
  • Verify patient eligibility, insurance coverage, and required authorizations.
  • Coordinate timely admissions to home health services.
  • Ensure referrals meet clinical and regulatory requirements.
  • Communicate with referral sources, physicians, clinicians, and patients to facilitate a smooth admission process.
  • Collaborate with the Billing and Authorization teams to ensure accurate documentation.
  • Maintain compliance with agency policies and state and federal regulations.

Compensation- $100,000-$120,000

Qualifications

  • Active Registered Nurse (RN) license required.
  • Bilingual and able to communicate in Spanish
  • Previous experience in home health, intake, case management, or care coordination preferred.
  • Strong organizational and communication skills.
  • Ability to multitask in a fast-paced environment.
  • Experience with insurance verification and authorizations is a plus.
  • Detail-oriented with excellent clinical judgment.
