Role Details

Job Title: Disability Navigator

Role Type: Max term contract, full time (38hrs per week until October 2027)

Role Grade: Dietitian, Occupational Therapist, Physiotherapist, Social Worker, Speech Pathologist (Level 3, Year 1-2)

Department and location: Disability and NDIS, Allied Health, St Vincent's Hospital, Darlinghurst

Salary: $116,892.00 - $120,680.00 + Super + Salary Packaging (New FY26/27 wage increase to be announced soon. All approved increases will be backdated to 1 July once implemented.)

Responsibilities
  • Lead the development and delivery of NDIS application and planning processes across SVHS, supporting People with Disabilities (PWD) to access appropriate supports and services.
  • Partner with PWD, families, carers, treating teams and key stakeholders to support NDIS applications, planning processes and the development of effective NDIS plans.
  • Develop, implement and evaluate NDIS application and planning guidelines, policies and procedures in consultation with the SVHS Disability Manager and relevant staff.
  • Monitor, analyse and report on NDIS application and planning data to improve, standardise and evaluate processes, including the development of KPIs and key milestone timeframes.
  • Liaise with internal and external stakeholders, including NDIA, Local Area Coordinators (LACs), clinical teams and other services, to address NDIS eligibility enquiries and support effective outcomes.
  • Develop education and training resources, facilitate information sessions, and lead quality improvement initiatives to enhance staff capability and NDIS processes.
Qualifications
  • Postgraduate qualifications or bachelor's degree in social work, dietetics, occupational therapy, physiotherapy or speech pathology and eligible for full membership of relevant Association along with evidence of clinical experience in a hospital, health, disability services or related field as a practicing clinician (in line with area of professional qualification).
  • Demonstrated advanced clinical reasoning, experience and practice skills relevant to assessment and treatment of people with a disability and/or functional impairment, including a high level understanding of the NDIS.
  • Demonstrated independent professional judgement in solving problems and managing cases where procedures and methods require expansion or adaptation.
  • Strong communication, negotiation and problem solving skills, with experience in clinical teaching, supervision and stakeholder engagement.
  • Ability to initiate, lead, complete and share quality improvement activities or service evaluation processes.
  • Proficient written, verbal and interpersonal communication skills with the proven ability to liaise with novice and experienced health professionals and external service providers.
  • Computer literacy and ability to use health information systems.
