What You’ll Do

  • Provide compassionate hospice nursing care to patients and families
  • Educate patients and caregivers on the hospice process and plan of care
  • Guide families through goals-of-care discussions and healthcare decisions
  • Conduct initial and ongoing comprehensive assessments
  • Perform hospice evaluations and informational visits
  • Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members
  • Coordinate care to ensure patient and family needs are fully met
  • Communicate changes in patient condition and update plans of care as needed

What You’ll Need

  • Active Missouri or Compact RN license
  • Hospice or Long-Term Care nursing experience preferred
  • Strong assessment, communication, and care coordination skills
  • Valid driver’s license and auto liability insurance
