Job Description

Direct Support Professional, Community Habilitation

Reports to: Community Habilitation Program Manager

Location: Onondaga County

Schedule: Various Part-Time Schedules Available

Pay: $19.49 - $21.20 Hourly

Offering person-centered services that empower individuals of all ages and abilities to reach their full potential as part of our community. Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.

About the Role:

We are looking for a proactive employee promote participant growth and independence while out in the community. Examples of some activities you will enjoy with our participants are: Going to the YMCA, Destiny USA Mall, state parks, bowling alleys, the zoo, and many more! Day to day, you will support our participants who enjoy exploring the community. Days are often spent participating in recreational activities like going to the movies, visiting libraries, parks, restaurants, and the mall. You will also support them with daily living activities such as grocery shopping, cooking, and laundry to help build independence and social interaction skills.

We have various shifts available for this position such as: Monday-Thursday 4pm-8pm, and Saturday 12pm-8pm | Monday 9am-1pm, Tuesday 9am-4pm, Wednesday 1pm-5pm, Friday 9am-1pm | Monday 9am-1pm, Tuesday 12pm-5pm, Thursday 9am-4pm, Friday 12pm-4pm. Our hours in community habilitation can be very flexible based on availability and participant needs!

Responsibilities:

  • Supporting participants with activities of daily living (ADL)
  • Implementing participants behavior guidelines and goals as written in Staff Action Plan
  • Monitoring health and safety, building advocacy, decision making, and community connections
  • Teaching participants skills to promote independence

Education/Experience:

High school diploma/GED preferred. Experience working with people with disabilities preferred. Basic word processing and e-mail skills are required.

Conditions of Employment

Must possess a valid driver’s license and transportation necessary to meet program needs. Must be able to lift 75 pounds.
