This job location is not currently located in a Health Professional Shortage Areas (HPSAs) and/or Medically Underserved Areas/Populations (MUA/P) Shortage Designation Type.

• Join an established team of 1 Physician and 1 Nurse Practitioner

• Each provider has 1:1 Medical Assistant support

• Remote RN triage support available through a regional pool

• Physicians take their own call Monday–Thursday nights

• Weekend call rotation: 1:7 (increasing to 1:8 in November 2026)

• MAs perform lab draws onsite

• Beautiful facility is approximately 6 years old

• Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community.

• Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning.

• Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system.

• Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice.

• Ability to practice autonomously and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment

Licensure / Certification / Registration:

• Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date required.
• Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education:

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
