Job Description

LPN - Long Term Care - Pay Rate: $40.00+ per hour - Day/Evening Rotating Shifts - 26 week contract

WorkFit Medical Staffing is seeking compassionate and dedicated Registered Nurse (RN) to join our team in providing exceptional care in long-term care facilities. Position Overview:

    • Perform comprehensive nursing assessments and monitor residents for changes in condition.
    • Develop, implement, and evaluate individualized care plans.
    • Administer medications, treatments, and other nursing interventions safely and accurately.
    • Monitor vital signs, symptoms, laboratory results, and overall health status.
    • Respond promptly to emergencies and changes in residents’ conditions.
    • Communicate significant findings and changes in condition to physicians and other healthcare providers.
    • Supervise and collaborate with Licensed Practical Nurses (LPNs/LVNs), Certified Nursing Assistants (CNAs), and other care staff.
    • Provide direction and support to nursing staff while maintaining appropriate delegation and supervision.
    • Coordinate admissions, transfers, discharges, and follow-up care.
    • Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
    • Educate residents and families regarding medications, treatments, disease management, and ongoing care.

  • Current and valid NY State RN license.
  • Minimum of 1 year experience in long-term care nursing preferred.
  • Basic Life Support (BLS) certification.
  • Strong clinical skills and attention to detail.
  • Effective communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
  • Compassionate and patient-centered approach to care delivery.
