- Must have 1 year of OT experience within the last 5 years
- Must have pediatrics experience
- Must have ortho experience
- Must have Epic experience
- Active and unencumbered California OT License
- Active BLS Certification (AHA only)
- Annual Physical within last 12 months
- Negative TB within last 12 months
- Hep B Series or Titer
- MMR Series or Titer
- Tdap Vaccine within last 10 years or Titer
- Varicella Series or Titer
