Please note that this role is per diem with a rate of $130 per visit.
Responsibilities
- Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
- Develop personalized treatment plans and communicate them to physicians and the care team
- Teach and retrain patients in daily living skills affected by illness or injury
- Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
- Maintain accurate clinical records and promptly report changes in patient condition
- Coordinate with the broader care team to ensure seamless, patient-centered care
- Educate family members and caregivers on the patient’s treatment approach
- Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
- Support discharge planning as patients transition out of home health care
- Perform other duties as required for patient services
- Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
- Registered with the National Registration Examination of the American Occupational Therapy Association
- Active California Occupational Therapy license
- Minimum 2 years of clinical OT experience
- Current CPR certification with AHA
- Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
- Strong communication and organizational skills
