Job Overview

 

Our client is seeking an Intermediate-level Social Worker to provide support services ensuring quality care for consumers receiving long-term living services at a residential care facility in Erie, PA. This is a fully onsite role with a flexible shift schedule.

Key Responsibilities

 

• Conduct on-site monitoring reviews of service providers to verify compliance with State and Federal guidelines
• Review written documentation and meet with consumers and stakeholders to evaluate service quality
• Monitor issues related to consumers' medical and financial eligibility
• Participate in statewide travel, including occasional overnight stays

Required Skills

 

• Bachelor of Social Work (BSW) from an accredited Social Work program, or equivalent education and experience
• Basic computer skills including proficiency in Microsoft Office products
• Ability to work independently with occasional expert guidance

Preferred Skills

 

• Experience in long-term care or community-based services
• Familiarity with State and Federal long-term living regulations

Location & Work Model

 

Fully onsite at Soldiers and Sailors Home, Erie, PA. Statewide travel required with occasional overnight stays.

Engagement Details

 

Flexible shift schedule available: Tuesday–Saturday or Monday–Friday. Shift options: 7:30 AM–3:30 PM, 8:00 AM–4:00 PM, 9:00 AM–5:00 PM, or 10:00 AM–6:00 PM.
