Freedom at Home Dialysis, Expedient Home Health Care and Green Hills Hospice are sister agencies. We are currently looking for a experienced Physical Therapist join our Home Health Team. Expedient Home Health provides comprehensive home health services to our coverage area.
Clinical Assessment & Care Planning
Conduct initial physical therapy evaluations in patient homes, including comprehensive functional assessments, range of motion, strength, balance, transfer ability, gait, pain, and home environment safety.
Establish measurable, goal-oriented Plans of Care (POC) tailored to patient needs and physician orders.
Perform timely Outcome and Assessment Information Set (OASIS) comprehensive assessments (start of care, re-certifications, discharges) in compliance with CMS guidelines.
Re-evaluate patient progress regularly and modify treatment goals or treatment frequency as clinical condition indicates.
Direct Patient Care & Intervention
Administer therapeutic exercises, gait and transfer training, balance activities, neuromuscular re-education, and manual therapy techniques.
Assess safety and recommend appropriate durable medical equipment (DME), assistive devices (walkers, canes, wheelchairs), and home modifications (grab bars, ramp access).
Educate patients, family members, and primary caregivers on home exercise programs (HEPs), body mechanics, fall prevention, and safety strategies.
Documentation & Coordination
Complete clinical documentation (evaluations, daily visit notes, discharge summaries, OASIS data) within agency-mandated timeframes (typically within 24 to 48 hours).
Collaborate with interdisciplinary team members, including physicians, registered nurses (RNs), occupational therapists (OTs), speech-language pathologists (SLPs), medical social workers (MSWs), and home health aides.
Participate in case conferences, care coordination meetings, and clinical quality reviews.
Supervise Physical Therapist Assistants (PTAs) in accordance with state licensing standards and agency supervision guidelines.
EXPERIENCE AND REQUIRED SKILLS
Qualifications & Requirements
Education: Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctor of Physical Therapy / DPT degree).
Licensure: Current, unencumbered Physical Therapist license in the state of practice.
Certifications: Current Basic Life Support (BLS) / CPR certification.
Experience: 1+ years of physical therapy experience required; home health or acute care/rehab experience preferred. Working knowledge of OASIS documentation standards preferred.
Transportation: Valid driver’s license, reliable personal vehicle, and active auto insurance.
