Responsibilities

🩺 What You’ll Be Doing (AKA: Your Superpowers)

• Completing Start of Care (SOC) visits with confidence, compassion, and clinical excellence
• Performing assessments that actually reflect the patient’s needs
• Creating individualized care plans that set the tone for amazing outcomes
• Educating patients and families so they feel empowered, not overwhelmed
• Collaborating with a supportive team that values your judgment and expertise
• Documenting in the EMR (we keep it as smooth and painless as possible)
• Being the RN who makes patients say, “Wow… I feel better already”

• Active RN license in Michigan
• Strong assessment skills and clinical judgment
• A warm, upbeat personality that helps patients feel instantly at ease
• Home health experience is a plus — but we’ll train the right nurse
• Reliable transportation and solid communication skills
• A love for autonomy, flexibility, and meaningful patient relationships
