Essential Duties & Responsibilities:
- Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
- Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
- Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
- Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
- Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
- Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
Qualifications:
- Eligible to work in the United States
- Active Physician Assistant license in Illinois preferred
- Current Controlled Substance License preferred (required upon start date)
- Current DEA License preferred (required upon start date)
- NCCPA Certificate preferred
- Interest in working in Physiatry/Physical Medicine and Rehabilitation preferred
- Orthopedic Experience (nice to have)
- Physiatry Experience (nice to have)
- Skilled Nursing Experience (nice to have)
License/Certification:
- Certified Physician Assistant preferred
