Nurse Practitioner, Heart Success Transplant Coordinator- Atlantic Health Morristown Medical Center

Lead the coordination and delivery of comprehensive pre- and post-heart transplant care across inpatient and outpatient settings.

Manage transplant candidate evaluations, waitlist activities, and post-transplant follow-up to ensure timely, patient-centered care.

Collaborate with physicians, advanced practice providers, nurses, and subspecialty teams to optimize clinical outcomes and continuity of care.
