Job Description

We are seeking a Senior Specialist, Population Health Management to join our team supporting a statewide Medicaid data platform. The ideal candidate will have strong experience in managed care plans, population health management, data and quality platforms, and stakeholder engagement and a proven ability to translate stakeholder needs into actionable requirements, drive adoption, and deliver measurable outcomes aligned to PHM-CalAIM.

Responsibilities:

• Build relationships and facilitate ongoing dialogue with target stakeholder groups on PHM services to improve Medi-Cal member health and experience through whole-system, person-centered services.


• Engage stakeholders to surface unmet needs, map and document opportunities, and inform PHM service enhancements.


• Provide input into developing and modifying systems to meet stakeholder needs, including drafting business specifications.


• Participate in technical reviews and inspections throughout the project lifecycle.


• Develop recommendations to drive utilization and proficiency with the PHM service and increase benefit realization tied to PHM-CalAIM outcomes.


• Communicate complex technical concepts simply and effectively to diverse audiences, including clients and internal teams.

Required Skills & Qualifications:

• Bachelor's degree or equivalent combination of education and experience.


• 5+ years managing large-scale change or transformation initiatives.


• Experience working with or at Managed Care Plans, with knowledge of data, quality, and population health platforms.


• Demonstrated knowledge of California DHCS programs and initiatives for the Medi-Cal population.


• Experience interfacing across data and functional groups within an organization.


• Excellent presentation and stakeholder communication skills.

Preferred Skills:

• Knowledge of California Medicaid landscape, including MCP and BHP policy.


• Prior experience supporting PHM-CalAIM or similar statewide initiatives.
