Key Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Provide skilled nursing care and education to patients and their families
• Ensure compliance with all safety, infection control, and documentation standards

• Active Registered Nurse License valid in the state of practice
• Minimum of 1 year home health nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Valid driver’s license and reliable transportation
• Ability to work independently and manage caseload effectively
• CPR certification
• Pass drug screening and background checks as required
