TRAINING SPECIALIST

Sevita is seeking a Cognitive Retraining Specialist to join our Healthcare & Medical Services team. In this role, you will deliver person-centered cognitive rehabilitation to individuals with brain injuries and related conditions, supporting skills in memory, attention, problem-solving, and daily functioning. You'll collaborate with an interdisciplinary team to develop and implement individualized treatment plans, track progress, and adjust interventions as needed. Responsibilities

• Conduct structured cognitive screenings and functional observations to identify strengths, needs, and rehabilitation priorities.
• Develop and implement individualized cognitive retraining plans based on assessment data, person-centered goals, and interdisciplinary input.
• Provide one-on-one and small-group cognitive rehabilitation interventions in home and community-based settings.
• Teach and reinforce compensatory strategies, environmental modifications, and skill-building exercises to support attention, memory, executive functioning, and communication.
• Coach individuals and families in the use of memory aids, organizational tools, safety strategies, and problem-solving techniques in daily routines.
• Collaborate closely with an interdisciplinary team, including clinicians, case managers, and direct support staff, to coordinate integrated care plans.
• Document assessments, treatment plans, progress notes, and outcomes accurately and in a timely manner in electronic health records.
• Monitor and analyze progress data, adjust treatment approaches as needed, and participate in quality improvement and program development efforts.
• Provide education and support to families, caregivers, schools, and community partners on cognitive strategies and accommodations.
• Maintain professional boundaries, adhere to ethical and regulatory standards, and ensure safety and risk management during all services and community activities.
