HOME VISIT NURSE - MULTIPLE LOCATIONS - PRN - AS NEEDED

Pay Range:

  • $65 – 75 / Visit

Role Description

This is a part-time/PRN on-site role for a Visit Nurse at Elite Care Management. As a Home Health Visit Nurse, you will be responsible for providing compassionate and personalized care to patients in their homes. Your day-to-day tasks will include conducting assessments, administering medications, coordinating with other healthcare professionals, and educating patients and their families on care plans.

Responsibilities:

  • Provide comprehensive patient care during shifts, including catheterization, administering medication, and dressing changes.
  • Conduct thorough patient assessments and monitor vital signs to ensure safety and well-being.
  • Utilize EMR systems for accurate documentation and maintain compliance with health regulations.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Administer basic life support and respond to emergencies as needed.
  • Ensure a clean and safe environment for patients.
  • Ability to sit, stand, and kneel as needed to perform physical assessment
  • Ability to work independently and part of a team
  • Indiana Registered Nurse (RN) license
  • Experience preferred in home health care or a similar setting & Oasis Documentation
  • Strong communication and interpersonal skills
  • Knowledge of medical equipment and procedures
  • Compassionate and empathetic demeanor
  • Attention to detail and organizational skills
  • Valid driver's license
  • CPR Certification
