We are looking for caring, compassionate and motivated Support Workers to join our team at one of our specialist mental health residential care homes.

The Role

As a Support Worker, you will work as part of a dedicated care team to provide high-quality, person-centred support to our residents. You will encourage independence, promote wellbeing and help residents develop and maintain the skills they need to live as independently as possible.

You will build trusting relationships with residents and become an important part of their support network, while working closely with colleagues, healthcare professionals and other significant people in their lives.

Key Responsibilities

Your responsibilities will include, but are not limited to:

  • Providing person-centred support that promotes dignity, choice, independence and wellbeing.
  • Supporting residents with household, personal and group management tasks.
  • Encouraging residents to develop, maintain and build upon their existing skills.
  • Acting as a Key Worker, taking responsibility for planning and coordinating support for an allocated resident.
  • Building positive, professional and trusting relationships with residents.
  • Supporting residents to achieve their individual goals and aspirations.
  • Liaising effectively with healthcare professionals, families and other significant people involved in residents' care.
  • Assisting with the development, preparation, monitoring and review of care plans and personal plans.
  • Maintaining accurate and up-to-date records using the home's care management app.
  • Supporting residents to attend appointments, meetings and activities where required.
  • Attending and contributing to residents' house meetings.
  • Attending and actively participating in staff meetings and supervision sessions.
  • Administering or assisting with the administration of medication in accordance with company policies and procedures, maintaining accurate medication records.
  • Working flexibly according to the needs of the service and residents.
  • Maintaining a high standard of professional conduct and adhering to the home's philosophy, policies and procedures.
  • Completing all mandatory and role-specific training and development provided by Hatzfeld Care.
  • Working collaboratively with colleagues to ensure residents receive consistent, safe and high-quality care.

  • Experience working with people who have mental health conditions and/or dementia desirable
  • Must have full UK driving license
  • Mileage paid over 5 miles
