The Role

\n

Karias Health is building AI-powered healthcare products that reduce costs, improve member care, and scale across self-funded employer health plans.

\n\n

This is a dedicated US engineering hire and a foundational seat. Our engineering has been offshore-led to date. Among core solutions, you will define the architecture for our latest platform and help raise the engineering standards the team builds against, working alongside our Lead Engineer and mentoring our offshore engineers while staying hands-on. This is roughly 80% building and 20% leading — we are hiring an architect who codes.

We build with AI, not just on top of it. We expect AI-assisted development across the lifecycle - from writing specifications through implementation, testing, and code review - and we are looking for someone who already works this way and can raise the bar for how the rest of the team does.

You report to our Lead Engineer and work closely with the CPTO on platform direction.

What You'll Own

  • Technical leadership. Mentoring our offshore engineers, raising the bar on code quality, and serving as a technical point of accountability for what ships on the platform.
  • \n
\n\n
    \n
  • AI-assisted engineering in practice. Using AI tooling across the full development lifecycle — specification, implementation, test generation, and code review — and helping establish the guardrails that make that safe in a regulated environment.

What You'll Deliver

\n

The first ninety days establish the technical foundation for the early-2027 enterprise launch. 

\n\n

Day 30 

\n

Full assessment of the existing codebase and infrastructure, with a written point of view on what carries forward into the white-label platform and what does not. 

\n\n

Day 60 

\n

Tenancy model and data isolation approach decided, documented, and agreed with the Lead Engineer and CPTO. 

\n\n

Go - Live 

\n

Multi-tenant foundation in build, and engineering standards and CI/CD practice established with the engineering team.

\n

 

\n

Stack

\n

Ruby on Rails, Python, AWS (RDS, S3, Redshift, Glue, OpenSearch), RAG pipelines with HIPAA-eligible LLMs.

\n\n

Qualifications

\n\n

Strong 

\n
    \n
  • Approximately 7+ years of professional software engineering experience, with a track record of owning architecture end-to-end rather than implementing someone else's design 
  • \n
\n\n
    \n
  • Production Ruby on Rails experience — comfortable working in a mature Rails codebase and making structural decisions within it, including tenancy scoping, migrations at scale, and service boundaries 
  • \n
\n\n
    \n
  • Demonstrated fluency with AI-assisted development — you already use AI tooling for specs, implementation, testing, and review, and have a considered point of view on where it helps and where it does not 
  • \n

  • Strong Python and production AWS experience 
  • \n

  • Direct experience building multi-tenant SaaS, including tenancy, data isolation, and per-tenant configuration decisions you have lived with the consequences of

  • Demonstrated technical leadership — mentoring engineers, setting standards, running code reviews — without needing to move into pure management to do it.

  • Experience shipping LLM-powered product features to real users

  • Experience working with distributed or offshore engineering teams

  • Familiarity with HIPAA and SOC 2 in a build context
