Join us and be part of something bigger – helping to simplify health care one person, one family and one community at a time.\n\n\n\nRegistered Nurse Case Manager – Field in Suffolk, Brooklyn, Kings and Queens and surrounding counties, NY \n\nPosition Summary:\nThis position is a field‑based Registered Nurse (RN) role responsible for conducting member assessments for new NY Better Health enrollees, annual reassessments, semiannual reviews, and change‑in‑condition visits.

The nurse will complete 2–3 home visits per day, with all documentation expected within 24 hours of each assessment.\nThe role requires extensive travel (up to 75%) across assigned Suffolk and Surrounding Counties, NY and close collaboration with the Scheduler/CM Assistant who coordinates\nWork hours are Monday–Friday, 8:30 a.m.–5:00 p.m.

EST.\n\nKey Responsibilities:\n\n Initial Assessments — Conduct member assessments for new NY Better Health applicants.\n Reassessments — Perform annual, semiannual, and change‑in‑condition assessments per regulatory and plan requirements.\n Clinical Documentation — Submit complete, accurate documentation within 24 hours of each visit.\n Care Coordination — Communicate findings to interdisciplinary teams to support care planning.\n Travel & Scheduling — Maintain punctuality and reliability for 2–3 daily home visits coordinated by the scheduling team.\n Independent Work — Operate effectively in a remote/telephonic environment while collaborating with cross‑functional partners.\n\n\nRequired Qualifications: \n\n Registered Nurse (RN) with current, unrestricted state licensure in NY\n Must possess reliable transportation and be willing and able to travel up to 75% of the time.

Mileage is reimbursed per our company expense reimbursement policy\n Case Management Experience\n Strong clinical assessment, critical thinking, and care coordination skills.\n Effective written and verbal communication skills.\n Ability to work independently in a telephonic/remote environment while collaborating effectively with cross‑functional teams.\n\n\nPreferred Qualifications: \n\n UAS NY Experience/Certification\n Preferred Prior Assessment/Reassessment Nursing Experience\n Experience working with Medicaid and/or LTSS populations.\n Bilingual (Spanish Speaking) is a plus\n\n\nEducation:\n\n Associate’s degree in Nursing, Bachelor’s preferred\n\n\n\n\nAnticipated Weekly Hours40\n\n\nTime TypeFull time\n\n\nPay Range\nThe typical pay range for this role is:\n$66,575.00 - $142,576.00\n\nThis pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. \n
