Job Description
LSCHC and the Chum Center are collaborating on a grant from the Minnesota Department of Heath to improve access and delivery of coordinated, comprehensive medical, mental health services and promote housing stability for people with serious mental illness and/or co-occurring disorders who are experiencing homelessness. In addition to Chum, there will be collaboration with other community partners to support our community’s homeless population. This individual understands the root causes of homelessness, has work or lived experience helping individuals to access healthcare and housing resources, has cultural competency and a willingness to learn about the cultural needs of people, thrives in an unpredictable work environment and shares our commitment to end homelessness through action and advocacy.
Primary Responsibilities:
- Provide outreach to individuals who are experiencing homelessness.
- Approach people from a place of respect to build relationships and trust that will help facilitate participation in their recovery and stabilization.
- Work collaboratively with Chum and other community partners to make connections, build relationships and support individuals who are unhoused by connecting them with the appropriate services.
- Provide health education in a relevant and accessible manner, sharing information about available resources so that people are equipped with the tools to make informed decisions about their health and well-being.
- Provide care recommendations and assist with making appointments for medical, dental, and recovery services, picking up prescriptions, transportation and follow-through.
- Establish and maintain a working relationship with health care, law enforcement, and emergency services professionals in the community; develop knowledge of available resources.
- Complete accurate case notes, log all contacts and protect clients’ private data.
- Collect and report National Client Level Outcomes (NOMS) for clients served by this project.
- Understand and adhere to mandated reporting responsibilities.
- Other duties as assigned.
Qualifications:
- Associates degree in a relevant field such as Social Work, Human Services, Family Studies, Medical Assistant, or Community Health worker Certification or willingness to get certification within 1 year of hire.
- Up to one year’s work or lived experience helping individuals to access healthcare resources is highly valuable.
- General knowledge of Health Care System, Mental Health First Aid and Trauma Informed Care.
- Hold a valid driver’s license and pass a background check.
- Be able to maintain composure in crisis.
- Be able to work independently, with a team and with your supervisor.
- Be able to maintain privacy and confidentiality.
- Be able to communicate clearly, professionally and compassionately with participants, providers, coworkers, and volunteers.

- Varied work hours. Days, some weekends and nights.
