Job Summary

We are seeking an experienced Outpatient Oncology Pharmacist to provide clinical pharmacy services in a hematology/oncology outpatient setting. The pharmacist will support the safe and effective use of chemotherapy and other medications while working closely with the healthcare team, patients, and families.

This position involves chemotherapy order processing, treatment-plan review, medication monitoring, clinical assessment, and patient education. The position does not involve anticancer agent dispensing, but requires expertise in treatment-plan building, order verification, patient monitoring, anticancer-agent dose modification, drug information, and patient education.

Key Responsibilities

• Provide clinical pharmacy services for patients in the Hematology/Medical Oncology Clinic.
• Review and process chemotherapy and other medication orders.
• Build and verify treatment plans according to established protocols.
• Monitor patients receiving chemotherapy and other oncology medications.
• Identify, solve, and prevent medication-related problems in the hematology/oncology population.
• Provide patient and family education regarding the safe and effective use of chemotherapy and other medications.
• Perform clinical dosing and therapeutic monitoring.
• Support medication preparation and dispensing processes as required.
• Collaborate with physicians, nurses, and other members of the healthcare team.
• Provide drug information and medication-related guidance.
• Maintain compliance with applicable oncology and medication-safety standards.

Required Qualifications

• Minimum 1 year of experience.
• Active Wisconsin or Compact Pharmacy License.
  • Pending Wisconsin/Compact license may be accepted at submission.
  • License must be obtained during compliance.
• BLS and ACLS certifications must be active and available at submission.
• NCCN certification may be pending at submission but must be completed during compliance.
• Epic experience is preferred.
• Teaching hospital experience is preferred.
• Community hospital experience is preferred.
• Travel experience is not required.

Important Facility Notes

• Candidates should preferably be submitted to only one opening per facility.
• The facility does not accommodate travelers with latex allergies.
• Travelers may return to the facility only after being separated for a minimum of one year.
• Modules are generally completed during orientation and average 3 4 hours.
• Module completion is tracked through the facility report.
