Sozialpädagoge / Sozialarbeiter (m|w|d) | Altötting, Germany | Hybrid

The Role

  • Provide psychosocial support to patients and their families throughout their hospital stay.
  • Clarify and assist with social security benefits and entitlements, such as health and nursing insurance.
  • Prepare patients and families for discharge, coordinating necessary home care and support services.
  • Inform patients and families about the organization of partial and full inpatient care.
  • Initiate outpatient and inpatient rehabilitation measures.
  • Advise patients and families on advance directives, healthcare proxies, and guardianship.

What You'll Need

  • Completed Bachelor's degree in Social Pedagogy or Social Work.
  • Ideally, several years of professional experience in a social services role.
  • Experience in organizing and arranging outpatient, partial, and full inpatient care, as well as social and economic support services.
  • Strong knowledge of social law and a collaborative spirit for team-based work.
  • Proficiency in standard office software and clinical information systems.
  • Excellent organizational skills combined with high social competence and empathy.
