JOB DESCRIPTION Job DescriptionThe role of the rehabilitation coordinator is to support patients at RNSH and Ryde Hospital in gaining timely access to appropriate rehabilitation services. This includes InReach, inpatient and outpatient rehabilitation services at public, affiliated or private units within NSLHD . The position will ensure daily coordination of patient flow and admissions to inpatient units, Graythwaite Day Rehabilitation (GDR) at Ryde hospital and InReach Rehabilitation at RNSH. The coordinator will provide leadership, clinical advice, consultation, education and support of other professionals in relation to rehabilitation. The incumbent will work closely with the RNSH and Ryde Heads of Department Rehabilitation Medicine as well as the Manager Allied Health, Rehabilitation, Aged Care and Ambulatory Care Services Ryde. The coordinator will collaborate with allied health, medical and nursing clinicians and develop effective linkages and networks with relevant internal and external stakeholders and services.

Candidates will need to meet the following criteria:

- Allied Health qualifications recognised for registration or membership with the relevant Australian professional board or AASW. For relevant Allied Health professions, must have current registration with (AHPRA) Australian Health Practitioner Regulation Agency. Current clinical experience at a minimum Level 2.
- Demonstrated leadership skills in a clinical environment with a focus on patient safety and evidence-based practice and knowledge of professional and clinical issues relating to rehabilitation across the health care spectrum.
- Extensive experience providing direct clinical interventions, case management and consultancy to clients participating in the rehabilitation process
- Demonstrated advanced level interpersonal and communication skills, including written, verbal, presentation skills; ability to produce high level reports, plans and briefings. as well as proficient computer literacy and an ability to confidently use health information systems.
- Demonstrated ability to work independently and as part of a team with a commitment to a multidisciplinary approach to care

Salary per hour:
Allied Health Professional Level 5: $68 .53 - $70.19
Clinical Nurse Consultant Grade 2: $75.35 - $76.86
