Supported Living Planner & Admin Specialist

UnitingCare Queensland is seeking a Supported Living Facilitator - Planning and Admin in Annerley. The role supports people living with disability to achieve meaningful daily living outcomes through planning, coordination and administrative tasks in a compassionate, compliant environment.

You'll develop and review individual plans, assist with community engagement and referrals, and perform data capture and reporting to support high-quality services.
