Job Description

Health Link Home Health & Hospice

San Francisco, CA (HQ-based, with regular travel to branches in San Francisco, the East Bay, and the South Bay) | Full-time | Reports to [CEO/Administrator] $200,000–$250,000 per year, DOE, plus a discretionary annual performance bonus

The role

For over 20 years, Health Link Home Health & Hospice has served the Bay Area with strong clinical systems and consistent growth. We're hiring a DPCS to own clinical performance across our Bay Area branches — not just administer it.

You'll be the senior clinical leader for Home Health: setting agency-wide standards, supervising branch Clinical Managers, and owning quality, compliance, and patient outcomes. It's a role required by California licensing regulations, with the authority to match.

What you'll own

  • Agency-wide clinical direction, standards, and goals across all branches
  • Direct supervision of branch Clinical Managers, including hiring, development, and performance
  • Clinical outcomes and quality metrics — rehospitalization, patient satisfaction, and appropriate utilization consistent with each patient's plan of care
  • Compliance with the Medicare Conditions of Participation, California licensing, and accreditation standards
  • QAPI, survey readiness, audits, and Plans of Correction
  • A clear line of sight for executive leadership into clinical performance and risk

What you'll need

  • Active California RN license in good standing
  • Qualification under 22 CCR § 74703(b): an RN with a bachelor's or higher degree in nursing or a health-related field plus three years' experience in the last five years in a home health agency, clinic, or health facility (one year supervisory or administrative); or an RN with four years' such experience in the last five (one year supervisory or administrative)
  • Current BLS/CPR certification
  • Working knowledge of the Home Health CoPs, California requirements, and OASIS
  • Valid California driver's license and reliable transportation, or another reliable way to travel between branches

Preferred: 5+ years of home health clinical leadership; multi-site experience; prior ownership of agency-wide quality metrics.
