Service Delivery Manager Full-timeTRACKS Care is an Aboriginal-owned NDIS provider based in Darwin. We deliver inclusive, person-centred supports to Territorians of all backgrounds. We specialise in supporting Aboriginal and Torres Strait Islander people with disability and build everything around real goals and real people.
You'll be the operational backbone of TRACKS Care. You are not a desk role, but a hands-on manager who ensures participants are supported, workers are rostered and looked after, services run as promised and issues are resolved before they become problems. This role reports directly to the Managing Director.
Key Responsibilities- Service delivery across daily personal activities, community access, household tasks, life-skills development, transport and support coordination
- Leading, rostering and developing the support-worker team
- Managing participant relationships - from intake and service agreements through to ongoing reviews and family communication
- Ensuring quality and compliance with NDIS Practice Standards, incident reporting, documentation and safeguarding
- Tracking operational performance - billable hours, participant numbers, utilisation and keeping the MD informed of ground-level activity
- 4+ years in disability services, with at least 2 years in a leadership role
- Proven track record of leading support workers and getting the best out of a team
- Genuine cultural competence, especially with Aboriginal and Torres Strait Islander people and communities
- Strong ability to manage competing priorities while prioritising participant safety
- Excellent communication skills across participants, families, workers and management
- Accountability, integrity and alignment with our values
- Confidence rostering within the SCHADS Award framework
Desirable but not essential:
- Tertiary qualification in disability, community services, social work or a related field
- Experience with shift-based rostering in disability or community care
- Experience working in or with NT communities
- NDIS Worker Screening Check (or willingness to obtain prior to commencement)
- Working with Children Check (or willingness to obtain prior to commencement)
- Current, unrestricted Australian driver's licence
- First Aid Certificate (or willingness to obtain prior to commencement)
- Legal right to work in Australia
