The Role

• Provide psychosocial support to patients and their families during their treatment.
• Manage and facilitate applications for social security benefits, including health and nursing insurance.
• Coordinate patient discharge planning in collaboration with patients and families.
• Organize and arrange necessary home care services and support.
• Inform patients and families about options for partial and full inpatient care.
• Initiate outpatient and inpatient rehabilitation measures.
• Advise patients and families on topics such as advance directives and guardianship.

What You'll Need

• Completed Bachelor's degree in Social Pedagogy or Social Work.
• Prior experience in a social services role within a healthcare setting is preferred.
• Familiarity with organizing and coordinating various care settings and support services.
• Solid understanding of social legislation and a strong team player mentality.
• Excellent organizational skills, high social competence, and empathy.
• Proficiency in relevant office software and clinical information systems.
