Social Worker Case Management

• Location: Jacksonville, FL
• Facility: Ascension St. Vincent's Riverside Hospital
• Schedule: Day shift | Part-time

• Drive end-to-end patient care from admission through discharge, prioritizing family values.

• Monitor progress continuously via clinical assessments, chart reviews, and evaluations.

• Eliminate care barriers and secure key resources to optimize outcomes.

• Align healthcare teams and leadership to ensure seamless plan execution.

• Guide documentation standards, post-acute options, and associate oversight.

Licensure/Certification/Registration:

• One or more of the following required:
  • Clinical Counselor obtained prior to hire date or job transfer date.
  • Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.
  • Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.

Education:

• Master's degree in Social Work required.

Work Experience:

• 3 years of experience preferred
